Help for Hair Pullers: Understanding and Coping With Trichotillomania

This is Part 5 of the What Causes Thinning Hair Series.

Trichotillomania is a psychological disorder in which people have an overpowering urge to pull out their hair. For some hair pulling may be intentional while for others it is an unconscious behavior. The hair pulling can be from their scalp, eyebrows, hair from the legs, arms, pubic area or other areas of their body. The majority of people pull hair with their fingers. However, some may utilize tools such as tweezers, brushes and combs. Moreover, a large amount of people with trichotillomania pull hair from their scalp, particularly in the crown area. Hair pulling from the scalp often leaves them with patchy bald spots on their head. Due to the irregularity of the bald spots, these people do all they can to cover up those areas.
The cause of trichotillomania is unknown, but is being researched on a continuous basis.

Signs and symptoms of trichotillomania include:
- Patchy bald areas on the scalp or other areas of your body
- Bare or missing eyelashes or eyebrows
- Chewing or eating pulled-out hair
- Playing with pulled-out hair
- Rubbing pulled-out hair against your lips or face

Treatment

Treating Trichotillomania: Cognitive-behavioral Therapy for Hairpulling and Related Problems

Trichotillomania was not documented or accepted as a disorder until the late 1980s. For this reason there isn’t much research about treatment. However, there are studies that show that some experts use two main treatments, cognitive behavior therapy (CBT) and medications.

Cognitive Behavior Therapy (CBT) – this therapy focuses on undoing the habit by teaching a person to become more conscious of pulling. Additionally, you are taught to be cognizant of the behaviors or emotions that set off the hair pulling and how to replace it with other less self inflicting behaviors.

Medications – there are some doctors who treat the disorder with antidepressants, however, like any drug they can have bad side effects. Many experts would say that medications is not a good treatment alone, but is paramount when used in conjunction with CBT.

At our El Paso, Texas practice, we offer hair restoration surgery and transplants to help our patients regain their confidence. We use a state-of-the-art micro-hair transplantation technique that has replaced methods of the past that used “plugs” or larger grafts of hair, which were often noticeable. With our modern techniques, no one will know you’ve had hair restoration surgery because the transplants are so small that they’re practically undetectable.
